F-1 and J-1 student requests must be submitted through your I3S Terra Dotta Student Portal. This includes CPT, OPT, RCL, Extensions, Travel Signatures, J-1 work authorizations, J-1 Academic Training and all other student requests.

F-1 and J-1 student request forms on this site are in the process of being removed or replaced. Paper or PDF forms will no longer be accepted.
